Clinical Pharmacist - Webster, New York

A direct hire opportunity is available for an experienced Home Infusion Staff Pharmacist to join a healthcare team in Webster, NY. This role focuses on providing expert pharmaceutical care in a home infusion setting, supporting patients with complex medication regimens, and ensuring compliance with established safety and quality standards.

Schedule:

• Monday through Thursday
• 8:00 AM to 5:00 PM

Key Responsibilities:

• Review and verify infusion orders and patient therapies to ensure accuracy and safety
• Participate in clinical rounds, collaborating with multidisciplinary teams to optimize patient care
• Perform sterile compounding and review total parenteral nutrition (TPN) therapies
• Maintain thorough patient records and individualized care plans
• Ensure compliance with USP guidelines and cleanroom standards
• Train and provide ongoing support to pharmacy staff
• Identify and implement operational improvements to enhance pharmacy services

Qualifications / Desired Experience:

• Current licensure as a Registered Pharmacist in the State of New York
• Minimum two years of recent acute care pharmacy experience, or at least one year of home infusion therapy experience preferred
• Strong knowledge and hands-on experience with intravenous admixture services and intravenous therapy
• Proven ability to evaluate and monitor drug therapy, taking into account patient conditions and collaborating effectively with physicians, nurses, and other healthcare professionals
