Key Responsibilities:

• Dispense and manage inpatient medications accurately and efficiently.
• Provide pharmacokinetic dosing guidance, particularly for vancomycin.
• Manage warfarin therapy monitoring and adjustments.
• Collaborate with healthcare teams to ensure optimal patient outcomes.
• Utilize the electronic medical record system (EMR), specifically EPIC, for order verification and documentation.

Qualifications and Desired Experience:

• Active Ohio Pharmacist license in good standing.
• Minimum of one year inpatient hospital pharmacy experience.

• Familiarity with EPIC Willow Inpatient system is preferred, but not required.
