Provide essential support to patients transitioning from hospital to home as a Transitions of Care Pharmacist in a dynamic hospital environment. This role emphasizes medication education, counseling, and ensuring medication refills before patient discharge to promote adherence and improve health outcomes. Commonly implemented as part of a meds to beds program, this position plays a crucial role in helping patients maintain their medication regimens after leaving inpatient care.

This long-term contract opportunity is based in a major health system serving a broad community, including several outpatient clinics and hospitals. While the position is contract-based initially, candidates have potential pathways to join the health system directly as permanent staff over time.

Schedule & Hours:

  • 8-hour shifts
  • Day and afternoon shifts available

Qualifications / Desired Experience:

  • Licensed pharmacist with experience or interest in hospital inpatient pharmacy
  • Background in Transitions of Care or Ambulatory Care pharmacy preferred
  • New PGY1 residency graduates are preferred but candidates without PGY1 will be considered
  • Strong patient communication skills and ability to educate on medication use

Key Responsibilities:

  • Provide medication counseling and education to patients prior to discharge
  • Facilitate timely medication refills to support patient adherence
  • Collaborate with interdisciplinary teams to support safe medication transitions
  • Document and communicate medication plans as part of discharge process
