Medication Management Pharmacist Remote/Onsite | Direct Hire Potential
We're currently seeking Medication Therapy Management Pharmacists for a 13-week opportunity with potential for direct hire. This specialty MTM position offers the flexibility of onsite or remote work and is an excellent opportunity for an experienced pharmacist with strong clinical skills and multiple state licenses.
We are especially interested in pharmacists who are licensed in Louisiana plus multiple states from the list below.
Duties
Prepare and dispense medications according to established policies, procedures, and protocols.
Accurately interpret prescriptions and maintain complete patient medication profiles.
Monitor drug therapy for interactions, contraindications, allergies, and appropriate dosing.
Identify and report suspected adverse drug reactions.
Provide clinical consultation to practitioners and recommend appropriate therapeutic alternatives.
Ensure safe, appropriate, and cost-effective medication therapy.
Document clinical activities and interventions accurately.
Participate in medication-use reviews, quality improvement activities, and pharmacy inspections.
Supervise and verify the daily activities of pharmacy support personnel.
Ensure pharmacy services are completed accurately and efficiently.
Maintain medication storage areas and identify outdated or unusable medications.
Professionally communicate with patients and healthcare providers by phone and in person.

Requirements
Current, active Louisiana Pharmacist License is required.
Multiple active state pharmacist licenses required.
Prior retail or hospital pharmacy experience.
Must have a minimum of 5 state licenses from the targeted states, with strong preference for candidates holding multiple licenses.

Ability to obtain additional state licenses as needed, with a minimum expectation of 10 additional state licenses within 18 months of starting the role.
Strong clinical judgment, communication, organization, and attention to detail.
