Hospital Pharmacist - Lewistown, Pennsylvania

Responsibilities:
- Provide comprehensive central pharmacy services, including entering and reviewing medication orders for accuracy.
- Dispense medications accurately and promptly.
- Offer timely and precise drug information to healthcare teams.
- Document patient monitoring and evaluations through pharmacy computer systems and clinical surveillance.
- Oversee daily operations in sterile compounding, ensuring aseptic techniques and accurate preparation of compounded sterile drugs.
- Prepare high-risk medications, including narcotic infusions and cytotoxic chemotherapy agents.
- Perform drug therapy monitoring and pharmacist-managed consults according to established protocols.
- In ambulatory care rotations, facilitate and document drug therapy decisions through recommendations and collaborative practice agreements.
Qualifications:
- Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) degree.
- Completion of PGY1 residency or a minimum of three years recent clinical experience.
- Valid Pennsylvania pharmacist license or eligibility to obtain one.
- Experience with Meditech IT systems preferred.
Schedule: Primarily day shifts, 8-hour shifts, with some variable days and afternoons.
This position offers a competitive salary with a midpoint of $139,000 per year and potential relocation support.
