Travel Hospital Pharmacist in Milwaukee, WI

Work Schedule:

• 12-hour day shifts
• 7 days on, 7 days off rotation

Key Responsibilities:

• Review medication orders for drug therapy appropriateness, including dose, dosage form, regimen, therapeutic duplication, and drug interactions
• Provide pharmacokinetic and anticoagulation dosing guidance
• Deliver pharmaceutical care through clinical services such as medication reconciliation and discharge medication education
• Oversee pharmaceutical compounding activities in compliance with USP 797 and USP 800 regulatory guidelines
• Utilize EPIC for clinical documentation and workflow management

Qualifications / Desired Experience:

• Current Wisconsin pharmacist license or eligibility for a temporary Wisconsin license
• Minimum of 1 year hospital pharmacy experience
• Familiarity with US compounding standards and clinical pharmacy practices
• Strong attention to detail and clinical judgment
