Clinical Pharmacist Home Infusion

Location: Milwaukee, WI
Job Type: Contract-to-Hire
Schedule: MondayFriday | 8-Hour Shifts (Between 7:30 AM 6:00 PM)
On-Call Requirement: One evening per week and every fourth weekend

We are seeking an experienced Clinical Pharmacist to join a home infusion pharmacy team in Milwaukee, Wisconsin. This contract-to-hire opportunity is ideal for a pharmacist with strong sterile compounding expertise and a clinical background in home infusion therapy. The selected candidate will play a key role in ensuring safe, accurate medication preparation and delivering high-quality patient-focused pharmacy services.

Position Overview

The Clinical Pharmacist will be responsible for reviewing and verifying infusion therapy orders, supporting sterile compounding operations, and providing clinical oversight for patients receiving home infusion treatments. This role requires a pharmacist who is comfortable working in a fast-paced environment and committed to maintaining the highest standards of safety, compliance, and patient care.

Key Responsibilities
Review and verify medication orders for accuracy, appropriateness, and patient safety
Oversee and participate in sterile IV compounding activities in accordance with regulatory and quality standards
Prepare and check compounded infusion medications as needed
Provide clinical support and recommendations related to home infusion therapies
Collaborate with pharmacy staff and healthcare providers to ensure optimal patient outcomes
Maintain compliance with state, federal, and accreditation requirements
Support daily pharmacy operations and contribute to continuous quality improvement initiatives


Qualifications
Active, unrestricted Wisconsin Pharmacist license required
Minimum of two years of recent sterile compounding experience required
At least two years of recent home infusion pharmacy experience strongly preferred
Current knowledge of IV admixture preparation and sterile compounding best practices
CPR certification preferred
Strong clinical judgment, communication skills, and attention to detail
Ability to work independently while contributing to a collaborative team environment


Schedule & Commitment
Monday through Friday schedule with 8-hour shifts
Operating hours between 7:30 AM and 6:00 PM
Participation in an on-call rotation every fourth weekend and one evening each week
Candidates must be comfortable committing to a minimum six-month contract period before consideration for permanent employment
