Key Responsibilities:

• Provide comprehensive long-term care pharmacy services
• Ensure accurate dispensing and patient counseling in a specialty pharmacy environment
• Collaborate with healthcare teams to optimize medication therapy management (MTM)
• Maintain compliance with all safety, quality, and regulatory standards

Qualifications:

• Active Pharmacist license with relevant state credentials
• PharmD degree or certification in Medication Therapy Management (MTM)
• Immunization Certification and CPR Certification are highly preferred
• Strong attention to detail and patient-focused approach
• Experience in long-term care pharmacy preferred but not mandatory

Schedule:

• Monday through Friday, 8:30 a.m. to 5:30 p.m.
• Some weekends may be required
