Home Infusion Pharmacist in Modesto, CA

A part-time Infusion Pharmacist opportunity is available to provide essential support 2-3 days per week, Monday through Friday from 8:30 a.m. to 5:00 p.m., starting as soon as possible with ongoing duration. This role is based in the Modesto/Ceres, CA area and is offered on a contract basis.

This position welcomes candidates who possess strong IV/Infusion clinical experience, with a particular preference for those who have prior home infusion pharmacy experience. Recent pharmacy graduates within the last 2-3 years who have gained IV/TPN experience during APPEs or internships are encouraged to apply, as training will be provided to build skills in working with IVs/TPNs in a hospital setting. Ideal candidates demonstrate excellent communication and teamwork skills, with a positive attitude that outweighs experience levels.

Key Responsibilities:

• Evaluate prescription orders for appropriateness regarding dosage, drug choice, administration route, and potential interactions.
• Maintain accurate patient records, including clinical history, lab results, and concurrent medications.
• Support compliance with regulations by managing documentation required by state and federal agencies and accrediting bodies.
• Order and assess clinical laboratory tests, collaborating with nurses and physicians to ensure medication safety and effectiveness.
• Provide clinical and technical support to physicians, nurses, and patients.
• Assist in policy implementation and supervise pharmacy staff including technicians and interns.
• Oversee sterile compounding and quality assurance programs.
• Document communications in patient medical records.
• Ensure pharmacy practices prioritize safety and provide drug therapy education to patients and caregivers.

Qualifications:

• Active California pharmacist license in good standing.
• Strong IV/Infusion and clinical pharmacy experience.
• Prior home infusion pharmacy experience highly preferred.
• Excellent communication and teamwork aptitude.
• Familiarity with CareTend system is a plus.
