Our client is looking for an experienced pharmacist to join their home infusion team in Rochester. If you have a background in acute care or home infusion and are looking for a clinically focused opportunity with a consistent schedule, this could be a great fit.

What Youll Do

• Review infusion orders and patient therapies

• Participate in clinical rounds

• Perform sterile compounding and TPN review

• Maintain patient records and care plans

• Ensure USP and cleanroom compliance

• Train and support staff

• Drive operational improvements

What Were Looking For

• Current licensure as a Registered Pharmacist in the State of New York.

• Minimum of two years of recent experience in an acute care setting or one year of recent home infusion therapy experience preferred.

• Experience and strong knowledge base in intravenous admixture services and intravenous therapy.

• Ability to evaluate and monitor drug therapy relative to the total needs and clinical conditions of home care patients in collaboration with physicians, nurses, and other healthcare professionals.
