About the Role
Our client is seeking an experienced Home Infusion Pharmacist to join their team in Rochester, NY. This is a great opportunity for a pharmacist with acute care or home infusion experience.

Key Responsibilities

• Review infusion orders and patient therapies
• Participate in clinical rounds
• Perform sterile compounding and TPN review
• Maintain patient records and care plans
• Ensure USP and cleanroom compliance
• Train and support staff
• Drive operational improvements
• Pharmacy System: CareTend

Qualifications

• Current licensure as a Registered Pharmacist in the State of New York.
• Minimum of two years of recent experience in an acute care setting or one year of recent home infusion therapy experience preferred.
• Experience and strong knowledge base in intravenous admixture services and intravenous therapy.
• Ability to evaluate and monitor drug therapy relative to the total needs and clinical conditions of home care patients in collaboration with physicians, nurses, and other healthcare professionals.
