Hospital Pharmacist - State College, PA

Take on a dynamic role as a Clinical Pharmacist, engaging in diverse clinical environments including the Emergency Department (ED) and Critical Care Unit (CCU), while contributing to an antibiotic stewardship team. Key Responsibilities:

• Accurately enter and verify medication orders; conduct thorough medication profile reviews for appropriateness.
• Deliver clinical pharmacy services tailored to specific patient populations.
• Provide drug information and participate in multi-disciplinary teams or rounds as assigned.
• Monitor drug therapy and manage pharmacist-led consults following established protocols.
• Actively participate in acute patient care scenarios, including cardiac arrest, stroke, sepsis, and trauma within ED and critical care settings.

Qualifications:

• Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) degree.
• Completion of a PGY1 residency or a minimum of three years of current clinical pharmacy experience.
• Licensed pharmacist in Pennsylvania or eligibility to obtain licensure.

Schedule:

• Primarily day shifts, 8-hour shifts.
• Variable workdays and afternoon shifts may be required.
