Hospital Pharmacist - State College, Pennsylvania
A rewarding opportunity is available for a Staff Pharmacist to engage in both inpatient and ambulatory care roles within a dynamic hospital environment in State College, PA. Key Responsibilities:
- Accurately review and enter medication orders, ensuring patient safety.
- Dispense medications promptly and precisely.
- Provide timely, evidence-based drug information to healthcare teams.
- Document clinical monitoring and evaluations through pharmacy systems and surveillance programs.
- Oversee sterile compounding activities, ensuring aseptic technique and accuracy.
- Prepare high-risk medications including narcotic infusions and cytotoxic chemotherapy drugs.
- Perform drug therapy monitoring and pharmacist-managed consults according to established protocols.
- Facilitate and document ambulatory care drug therapy decisions via recommendations and collaborative practice agreements when rotating in ambulatory settings.
Qualifications and Desired Experience:
- Bachelor of Science in Pharmacy or PharmD required.
- Completion of PGY1 residency or a minimum of 3 years clinical pharmacy experience.
- Licensed pharmacist in Pennsylvania or eligible to obtain licensure.
- Experience with Meditech system preferred.
- Strong clinical judgment and ability to adapt between inpatient and ambulatory care roles.
