Key Responsibilities:

  • Accurately review and enter medication orders, ensuring patient safety.
  • Dispense medications promptly and precisely.
  • Provide timely, evidence-based drug information to healthcare teams.
  • Document clinical monitoring and evaluations through pharmacy systems and surveillance programs.
  • Oversee sterile compounding activities, ensuring aseptic technique and accuracy.
  • Prepare high-risk medications including narcotic infusions and cytotoxic chemotherapy drugs.
  • Perform drug therapy monitoring and pharmacist-managed consults according to established protocols.
  • Facilitate and document ambulatory care drug therapy decisions via recommendations and collaborative practice agreements when rotating in ambulatory settings.

Qualifications and Desired Experience:

  • Bachelor of Science in Pharmacy or PharmD required.
  • Completion of PGY1 residency or a minimum of 3 years clinical pharmacy experience.
  • Licensed pharmacist in Pennsylvania or eligible to obtain licensure.
  • Experience with Meditech system preferred.
  • Strong clinical judgment and ability to adapt between inpatient and ambulatory care roles.
