This role centers on providing medication education and counseling to patients prior to discharge and ensuring their medications are appropriately refilled before leaving the hospital, commonly referred to as a meds to beds program. This proactive approach increases patient compliance with medication regimens and supports healthier outcomes.

Located in Sylmar, CA, this contract opportunity serves the Los Angeles County Department of Health hospital system, a large network encompassing multiple outpatient clinics and hospitals. Pharmacists in this position will work day and afternoon 8-hour shifts, offering valuable ambulatory care support within a high-impact healthcare environment.

Preferred candidates are recent graduates with PGY1 residency training, though non-PGY1 pharmacists are also considered. This long-term contract role is managed through an agency with the potential for transition to direct County employment. While some pharmacists have secured contract-to-hire status in as little as twelve months, timelines vary and are not guaranteed.

Key Responsibilities:

  • Educate and counsel patients on medication use and adherence before hospital discharge.
  • Facilitate medication refills to ensure patients leave with necessary prescriptions.
  • Collaborate with healthcare teams to support seamless transitions from inpatient to outpatient care.
  • Participate in Ambulatory Care programs to enhance continuity of care.

Desired Qualifications:

  • Active pharmacist licensure with hospital inpatient experience.
  • Strong communication and patient education skills.
  • Familiarity with medication management in transitions of care settings.
  • PGY1 residency completion preferred but not mandatory.
