Hospital Pharmacist | Sylmar, California

A hospital-based pharmacist position is available focusing on Transitions of Care and Ambulatory Care within a large county health system. This critical role supports patient health outcomes by ensuring smooth medication transitions at discharge through education, medication counseling, and refill assistancecommonly known as a meds to beds program. The goal is to improve adherence to medication regimens and promote patient well-being after leaving the hospital.

Key Responsibilities:

• Provide comprehensive medication education and counseling to patients prior to discharge.
• Facilitate timely medication refills to ensure patients have their medications upon leaving.
• Collaborate with healthcare teams within ambulatory care and hospital settings.
• Support the operational needs of the pharmacy department as part of a contract-to-hire model.

Qualifications and Desired Experience:

• Licensed Pharmacist with experience in hospital pharmacy practice.
• New PGY-1 residents strongly preferred; however, non-PGY-1 candidates will be considered.
• Background in retail or outpatient pharmacy settings is acceptable.
• Fluency in Spanish is highly regarded and considered a plus.
• Ability to work flexible 8-hour shifts including primarily first and second shifts, with occasional third shifts.

Schedule/Hours:

• 8-hour shifts
• Flexibility to cover all shifts, mainly 1st and 2nd shifts; occasional 3rd shift required

Location:

• Sylmar, CA

This role is a contract position with an opportunity to transition to permanent employment within the county health system. The contracting approach supports timely onboarding due to a prolonged permanent hiring process.
