About

Our client is looking for an experienced Home Infusion Pharmacist to join their team in Rochester, NY. This is an excellent opportunity for a pharmacist with a background in acute care, IV therapy, or home infusion who is looking to take on a clinically focused role supporting patients in the home-care setting.

What Youll Do

Review infusion orders and assess patient-specific therapies
Collaborate with physicians, nurses, and other healthcare professionals
Participate in clinical rounds and provide recommendations related to drug therapy
Perform sterile compounding and review TPN orders
Monitor patient therapy based on clinical needs and treatment goals
Maintain accurate patient records and individualized care plans
Ensure compliance with USP standards and cleanroom requirements
Train, mentor, and support pharmacy staff
Identify opportunities to improve pharmacy operations and patient care
CareTend pharmacy software

Qualifications

Current New York State Registered Pharmacist license
At least 2 years of recent acute care pharmacy experience or 1 year of recent home infusion experience preferred
Strong knowledge of intravenous admixture services and IV therapy
Experience evaluating and monitoring drug therapy in relation to the overall clinical needs of home-care patients
Strong communication and collaboration skills
Ability to work effectively with an interdisciplinary healthcare team
