RPh on the Go is seeking a Clinical Pharmacist for a 13-week contract assignment at an inpatient rehabilitation hospital in Boise, ID, with the possibility of extension. This role focuses on medication safety, clinical review, and collaboration with the healthcare team to support patient recovery.

Position Details:

• Employment Type: 13-week contract, with potential extension

• Location: Boise, ID

• Setting: Inpatient rehabilitation hospital

• Schedule: To be discussed

• EMR System: Ace-It (modified Cerner application)

Responsibilities:

• Review medication orders for accuracy, appropriate dosing, and potential interactions.

• Complete medication reconciliation and identify discrepancies in patient histories.

• Educate patients on medications, dosing, and potential side effects.

• Collaborate with physicians and nurses to resolve medication-related concerns.

• Identify medication safety risks and support appropriate treatment decisions.

• Assist with daily pharmacy operations and maintain accurate clinical documentation.

• Ensure compliance with hospital policies and pharmacy regulations.

Qualifications:

• Active pharmacist license in good standing.

• At least 1 year of hospital pharmacy experience preferred.

• Strong clinical knowledge and ability to collaborate with an interdisciplinary healthcare team.
