Home Infusion Staff Pharmacist in Cibolo, TX
Join a dynamic healthcare setting in Cibolo, TX, as a Sterile Compounding Pharmacist specializing in home infusion therapy. This 13-week contract position offers an engaging opportunity to apply your clinical expertise in a critical care area, working the 12 PM to 8 PM shift.
Responsibilities:
- Evaluate patient eligibility for home infusion therapy and develop individualized medication plans based on clinical and pharmacokinetic assessments.
- Prepare, verify, and dispense sterile medications according to USP standards, state and federal guidelines, and organizational policies.
- Educate patients, caregivers, and healthcare providers on medication therapy, infusion techniques, and safe medication use.
- Collaborate with healthcare professionals to ensure continuity of care and monitor patient outcomes.
- Manage infusion devices and medication administration equipment, maintaining quality assurance and safety protocols.
- Provide accurate drug information, maintain comprehensive documentation, and support compliance initiatives related to medication safety.
Qualifications:
- Doctor of Pharmacy (PharmD) degree or graduate of an accredited School of Pharmacy.
- Active, unrestricted Pharmacist license in Texas.
- One or more of the following:
- Minimum of 1 year sterile IV compounding experience,
- Experience in non-sterile compounding, immunizations, and Medication Therapy Management (MTM),
- Completion of an ASHP-accredited Pharmacy Residency,
- Completion of an Advanced Pharmacy Practice Experience (APPE),
- Equivalent relevant pharmacy experience.
- Strong clinical knowledge of home infusion pharmacy practices.
- Proficient in interpreting laboratory values and applying pharmacokinetic principles.
- Exceptional communication and patient counseling skills.
