Per Diem Home Infusion Staff Pharmacist - Clayton, NC
Provide expert pharmaceutical care as a Home Infusion Pharmacist, focusing on sterile compounding and patient-centered medication management in a home infusion pharmacy setting. This per diem role offers an opportunity to apply your specialized skills in ensuring safe and accurate medication delivery to patients outside the traditional hospital environment.
Location: Clayton, NC
Schedule/Hours:
- Monday through Friday
- 9:00 AM to 5:00 PM
- Coverage period: July 29 through August 5
Key Responsibilities:
- Verify pharmaceutical products entering the clean room for sterile compounding, ensuring adherence to batch records and lot numbers.
- Perform final checks on compounded sterile products (CSPs), including labels, fill amounts, and drug components.
- Review and approve shipments to patients, confirming all medications and medical supplies align with delivery documentation.
- Communicate effectively with physicians, home care nurses, and patients to discuss drug regimens, inventory, adverse reactions, and abnormal lab results.
- Proactively contact patients to coordinate medication deliveries, assess inventory, monitor compliance, and report issues to management.
- Handle incoming calls from medical providers, nursing agencies, and patients regarding medication therapy and supply needs.
- Utilize CPR+ software proficiently as part of daily operations.
Qualifications:
- Bachelor of Science in Pharmacy required; Doctor of Pharmacy or Master of Science in Pharmacy preferred.
- Active pharmacist license in North Carolina.
- Minimum two years recent acute-care experience, or at least one year recent experience in home infusion therapy.
- Demonstrated expertise in intravenous admixture services and intravenous therapy.
