Home Infusion Staff Pharmacist - Converse, Texas

This opportunity is for an experienced pharmacist to specialize in home infusion therapy with a focus on sterile compounding. Located in Converse, TX, this 13-week contract position offers a structured schedule of 12 PM to 8 PM, providing consistent evening hours.

Responsibilities include evaluating patients for home infusion eligibility and developing personalized medication therapy plans based on clinical evaluations, lab values, and pharmacokinetic data. You will prepare, verify, and dispense sterile medications adhering to USP standards and regulatory requirements while educating patients, caregivers, and healthcare providers on medication use and infusion administration. The role also demands close collaboration with healthcare professionals to ensure seamless care continuity, management of infusion devices, quality assurance, and strict compliance with medication safety protocols.

Qualifications:

• Doctor of Pharmacy (PharmD) degree or a graduate from an accredited School of Pharmacy
• Active, unrestricted pharmacist license in Texas
• One or more of the following: Minimum 1 year of sterile IV compounding experience Experience in non-sterile compounding, immunization administration, and Medication Therapy Management (MTM) Completion of an ASHP-accredited Pharmacy Residency Completion of an Advanced Pharmacy Practice Experience (APPE) Equivalent pharmacy experience
