Staff Pharmacist | Converse, Texas
A specialized opportunity is available for a Sterile Compounding Pharmacist focused on home infusion therapy. This 13-week contract role, based in Converse, TX, emphasizes expert clinical assessments, sterile medication preparation, and patient-centered care within a home health setting.
Key responsibilities include:
- Evaluating patient eligibility for home infusion therapy and developing individualized medication plans based on clinical data and pharmacokinetic principles.
- Preparing, verifying, and dispensing sterile medications in full compliance with USP standards and regulatory requirements.
- Educating patients, caregivers, and healthcare providers on medication administration and infusion techniques to ensure safe and effective treatment.
- Collaborating with healthcare teams to maintain continuity of care and monitor clinical outcomes.
- Managing infusion pumps and related equipment while upholding quality assurance and safety protocols.
- Providing drug information, maintaining thorough documentation, and supporting medication safety and regulatory compliance efforts.
Qualifications and Desired Experience:
- PharmD or graduate of an accredited School of Pharmacy.
- Active, unrestricted Pharmacist license in Texas.
- Minimum of one year of sterile IV compounding experience OR experience with non-sterile compounding, immunizations, Medication Therapy Management (MTM), completion of an ASHP-accredited residency, an Advanced Pharmacy Practice Experience (APPE), or equivalent pharmacy experience.
- Strong clinical knowledge of home infusion pharmacy practices, with the ability to interpret laboratory values and apply pharmacokinetic principles effectively.
- Excellent communication, patient counseling, and customer service skills essential.
Schedule:
- 12:00 PM to 8:00 PM
