A pharmacy professional role is available focusing on home infusion services, offering an opportunity to work Monday through Friday with rotating shifts between 8:00 AM and 6:00 PM. This position is contract to hire with a 13-week initial assignment and potential for permanent placement.

Location: Edmond, OK
Schedule:

  • Monday to Friday, 8:30 AM to 6:00 PM (rotating shifts)
  • Scheduled on-call shifts, approximately once a month, from Friday 5:30 PM to Monday 8:30 AM

Responsibilities include:

  • Collaborate closely with physicians, nurses, and healthcare professionals to evaluate and monitor home care drug therapy tailored to patient needs.
  • Ensure compliance with all organizational policies, state and federal pharmacy laws, and accreditation standards.
  • Coordinate patient referrals and admissions in partnership with nursing and customer service teams.
  • Review and verify new patient orders accurately and document accordingly.
  • Assist healthcare providers in planning patient care, establishing therapeutic objectives, and monitoring drug therapy effectiveness and safety.
  • Conduct and document ongoing patient assessments to identify and address drug-related problems.
  • Monitor laboratory and clinical results to evaluate drug therapy outcomes.
  • Provide expert consultation to patients, families, and medical professionals about medications and appropriate use.
  • Maintain complete and accurate documentation of clinical activities and communications.
  • Prepare and check all medications and supplies before dispensing.
  • Perform sterile compounding of mixtures such as parenteral nutrition and antibiotics, adhering strictly to aseptic techniques and USP 797/800 standards.
  • Maintain and sanitize compounding areas and equipment, ensuring compliance with safety protocols.
  • Engage in continuous professional development through literature review and educational events.

Qualifications:

  • Current Oklahoma pharmacy license.
  • Experience in sterile compounding.
  • Background in home infusion or inpatient hospital pharmacy practice.
