Sterile Compounding Pharmacist Home Infusion Therapy

This opportunity seeks a knowledgeable and detail-oriented Sterile Compounding Pharmacist to support home infusion therapy services. The role requires a proactive professional to evaluate patient eligibility for home-based care, manage individualized medication plans, and ensure the safe preparation and dispensing of sterile medications. Immediate availability to start is essential.

Key Responsibilities:

  • Assess patients for home infusion therapy and determine eligibility.
  • Develop, monitor, and adjust personalized medication regimens based on clinical data and pharmacokinetic principles.
  • Prepare, verify, and dispense sterile compounds in strict adherence to USP standards and regulatory guidelines.
  • Educate patients, caregivers, and healthcare providers on medication protocols and infusion administration.
  • Collaborate with healthcare teams to maintain continuity of care and monitor patient outcomes.
  • Manage infusion pumps and related equipment, ensuring adherence to quality and safety standards.
  • Provide drug information, maintain thorough documentation, and support compliance and medication safety initiatives.

Qualifications and Desired Experience:

  • Doctor of Pharmacy (PharmD) or graduate of an accredited School of Pharmacy.
  • Active, unrestricted Pharmacist license in Texas.
  • One or more of the following:
    Minimum 1 year of experience in sterile IV compounding, OR
    Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM), OR
    Completion of an ASHP-accredited Pharmacy Residency, OR
    Completion of an Advanced Pharmacy Practice Experience (APPE), OR
    Equivalent pharmacy experience.
  • Strong clinical knowledge of home infusion pharmacy practices.
  • Proficiency in interpreting laboratory values and applying pharmacokinetic principles.
  • Excellent communication, patient counseling, and customer service skills.
