Home Infusion Staff Pharmacist - Garland, TX

Join a dynamic healthcare environment as a Sterile Compounding Pharmacist dedicated to supporting home infusion therapy. This contract role requires immediate availability for a 13-week assignment, providing a unique opportunity to apply your clinical expertise in a hands-on setting.

Schedule:

• Monday through Friday
• 7:00 AM to 3:30 PM

Responsibilities:

• Evaluate patients for home infusion therapy eligibility and develop individualized medication plans based on clinical assessments, lab values, and pharmacokinetics.
• Prepare, verify, and dispense sterile medications adhering to USP standards and regulatory requirements.
• Educate patients, caregivers, and healthcare providers on medication therapy and infusion techniques.
• Collaborate with healthcare teams to ensure continuity of care and optimize patient outcomes.
• Manage infusion pumps and medication administration equipment, maintaining quality assurance and safety protocols.
• Provide drug information, maintain accurate documentation, and support regulatory compliance and medication safety initiatives.

Qualifications:

• Doctor of Pharmacy (PharmD) or graduate of an accredited School of Pharmacy.
• Active, unrestricted Pharmacist license in Texas.
• Must meet at least one of the following:
  • Minimum 1 year of sterile IV compounding experience.
  • Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM).
  • Completion of an ASHP-accredited Pharmacy Residency.
  • Completion of an Advanced Pharmacy Practice Experience (APPE).
  • Equivalent pharmacy experience.
