Seeking a skilled Sterile Compounding Pharmacist for a 13-week contract position in Helotes, TX. This role is essential for delivering exceptional home infusion therapy and medication management to support patient care in a home-based setting. The schedule is from 12 PM to 8 PM, offering a consistent work routine.

Key Responsibilities:

  • Evaluate patients to determine eligibility for home infusion therapy.
  • Develop and adjust individualized medication therapy plans based on clinical assessments, lab values, and pharmacokinetic principles.
  • Prepare, verify, and dispense sterile medications following USP standards and regulatory requirements.
  • Educate patients, caregivers, and healthcare professionals on medication protocols and infusion administration.
  • Collaborate with healthcare providers to ensure continuity of care and monitor patient outcomes.
  • Manage infusion pumps and medication administration equipment while upholding quality assurance and safety standards.
  • Provide drug information, maintain accurate documentation, and support compliance and medication safety initiatives.

Minimum Qualifications:

  • Doctor of Pharmacy (PharmD) degree or graduate of an accredited School of Pharmacy.
  • Active, unrestricted Pharmacist license in Texas.
  • One or more of the following:
    • Minimum of 1 year sterile IV compounding experience.
    • Experience in non-sterile compounding, immunizations, and Medication Therapy Management (MTM).
    • Completion of an ASHP-accredited Pharmacy Residency.
    • Completion of Advanced Pharmacy Practice Experience (APPE).
    • Equivalent pharmacy experience.

Required Skills:

  • Strong clinical knowledge of home infusion pharmacy practices.
  • Ability to interpret laboratory values and apply pharmacokinetic principles effectively.
  • Excellent communication and patient counseling skills.
