IV Sterile Compounding Home Infusion Staff Pharmacist | Mustang, Oklahoma

This position focuses on providing expert pharmaceutical care within a home infusion environment, collaborating closely with healthcare professionals to optimize patient outcomes. The role requires comprehensive knowledge of sterile compounding and drug therapy management tailored to home care patients.

Location: Mustang, OK
Schedule:

• Monday to Friday, 8:30 AM 6:00 PM
• Rotating shifts including 8:30 AM 5:30 PM, 8:00 AM 5:00 PM, and 9:00 AM 6:00 PM
• On-call shift approximately once a month from Friday 5:30 PM to Monday 8:30 AM

Responsibilities:

• Evaluate and monitor drug therapy for home care patients in collaboration with physicians, nurses, and other healthcare professionals.
• Ensure compliance with all organizational policies, state, and federal regulations regarding pharmacy practice.
• Participate in quality assurance and accreditation standards adherence.
• Coordinate new patient referrals and admissions by working with nursing staff and customer service.
• Review, verify, and document patient orders accurately.
• Assist healthcare teams in planning patient care, setting therapeutic objectives, and monitoring drug therapy safety and efficacy.
• Conduct initial and ongoing assessments to identify drug-related needs or issues.
• Monitor patient outcomes through laboratory and clinical data analysis.
• Provide drug-related consultation to patients, families, and healthcare providers.
• Maintain thorough clinical documentation and communication records.
• Compound sterile and non-sterile mixtures adhering to USP 797 and USP 800 standards.
• Maintain cleanliness and compliance of compounding areas and equipment.
• Continuously update clinical knowledge through literature review and professional development activities.

Qualifications:

• Active Oklahoma Pharmacy License.
• Experience in sterile compounding.
• Prior experience in home infusion or inpatient hospital pharmacy practice.
