This opportunity is for a skilled pharmacist experienced in home infusion and sterile compounding, working a Monday through Friday schedule with rotating hours between 8:00 AM and 6:00 PM. The position supports patient-centered care by collaborating closely with physicians, nurses, and other healthcare professionals to evaluate and monitor drug therapy for home care patients.

Key Responsibilities:

  • Evaluate and monitor drug therapy in home infusion settings alongside healthcare teams.
  • Coordinate new patient referrals and admissions in collaboration with nurses and customer service.
  • Review, verify, and document all new patient orders accurately.
  • Assist in planning and implementing patient care, setting therapeutic goals, and monitoring drug therapy efficacy and safety.
  • Conduct initial and ongoing assessments to identify real and potential drug-related problems.
  • Provide drug consultation and respond to inquiries from patients, families, and healthcare professionals.
  • Process and check medications, supplies, and equipment prior to dispensing.
  • Compound sterile and non-sterile mixtures, including parenteral nutrition and chemotherapy, following aseptic technique and compliance with USP 797 and USP 800 standards.
  • Maintain clean room and compounding areas according to policies and standards.
  • Keep thorough documentation of clinical activities and pharmacy communications.

Qualifications:

  • Valid Oklahoma pharmacy license.
  • Demonstrated experience with sterile compounding.
  • Background in home infusion or inpatient hospital pharmacy practice.
  • Strong knowledge of pharmacy laws, standards, and quality assurance.

Work Schedule:

  • Monday to Friday, with rotating shifts: 8:30 AM5:30 PM, 8:00 AM5:00 PM, or 9:00 AM6:00 PM.
  • Scheduled on-call shifts approximately once a month from Friday 5:30 PM to Monday 8:30 AM.
