A rewarding opportunity for a pharmacist skilled in sterile compounding and home infusion awaits in Oklahoma City. This contract-to-hire position offers a structured Monday through Friday schedule with rotating shifts and occasional on-call duties, perfect for professionals seeking to expand their clinical expertise in a home infusion environment.
Key Responsibilities:
- Collaborate closely with physicians, nurses, and healthcare teams to evaluate and monitor drug therapy tailored to home care patients.
- Ensure compliance with all organizational policies, state and federal pharmacy regulations, and accreditation standards.
- Facilitate new patient referrals and admissions in coordination with nurses and customer service.
- Review and verify new patient medication orders, documenting appropriately.
- Assist healthcare professionals with patient care planning, therapeutic goal setting, and monitoring parameters.
- Conduct comprehensive assessments to identify drug-related issues and monitor therapy outcomes through clinical data analysis.
- Provide expert consultation to patients, families, and healthcare providers about medications and their use.
- Prepare sterile compounded medications including parenteral nutrition, antibiotics, chemotherapy, and narcotics under aseptic technique per USP 797 and USP 800 standards.
- Maintain cleanliness and compliance of compounding areas and equipment.
- Keep thorough documentation of all clinical interventions.
- Stay current with pharmacotherapy advances through ongoing education and training.
Desired Qualifications:
- Valid Oklahoma pharmacy license.
- Experience in sterile compounding with a strong background in home infusion or inpatient hospital pharmacy.
- Commitment to quality assurance and adherence to regulatory standards.
Schedule:
- Monday to Friday, 8:30 AM 6:00 PM with some shift variations (8 AM 5 PM or 9 AM 6 PM).
- Scheduled on-call shifts approximately once a month from Friday evening through Monday morning.
