Per Diem Home Infusion Staff Pharmacist in Plainfield, IL

A healthcare facility is seeking a Per Diem Staff Pharmacist to provide expert coverage in a home infusion pharmacy setting. This role supports the delivery of IV and infusion services to patients with both acute and chronic medical conditions, ensuring safe and effective medication management. Training will be provided, making this an excellent opportunity for pharmacists looking to expand their clinical skills in a specialized pharmacy environment.

Location: Onsite in Orland Park, IL
Schedule:

• Weekdays only (MondayFriday), no weekends
• 9:00 AM 5:00 PM
• Coverage Period: November 1330, 2026

Key Responsibilities:

• Evaluate and process physician orders accurately and efficiently
• Perform sterile compounding following USP standards and support cleanroom operations
• Dispense medications and infusion supplies safely
• Maintain precise patient and pharmacy records in compliance with state and federal regulations
• Communicate effectively with patients, physicians, nurses, and other healthcare professionals
• Supervise pharmacy technicians and support staff
• Manage assigned patient caseload and provide pharmacist coverage as needed

Qualifications:

• Active Illinois Pharmacist License in good standing
• Minimum of 1 year pharmacy experience; 2+ years of clinical pharmacy experience preferred
• Experience in infusion therapy and sterile compounding is highly desirable
• Strong knowledge of USP sterile compounding standards
• Excellent communication, multitasking, time management, and critical-thinking abilities
