Home Infusion Staff Pharmacist in Plano, TX
This opportunity seeks a skilled pharmacist specializing in home infusion and sterile compounding to deliver exceptional patient care. The role requires evaluating patients for eligibility in home infusion therapy and tailoring individualized medication plans based on clinical assessments and pharmacokinetics. You will prepare and dispense sterile medications in strict compliance with USP standards and regulatory requirements while educating patients, caregivers, and healthcare professionals on medication use and infusion procedures.
Key Responsibilities:
- Assess patients for home-based infusion therapy and develop personalized treatment plans
- Prepare, verify, and dispense sterile medications adhering to all safety and regulatory guidelines
- Provide education on medication administration and infusion techniques to patients and healthcare teams
- Collaborate with healthcare providers to ensure continuity of care and monitor patient outcomes
- Manage infusion equipment and maintain quality assurance and safety protocols
- Maintain accurate documentation, support compliance efforts, and provide drug information as needed
Qualifications:
- Doctor of Pharmacy (PharmD) or graduate from an accredited School of Pharmacy
- Active, unrestricted Texas Pharmacist license
- Minimum of one of the following: At least 1 year experience in sterile IV compounding Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM) Completion of an ASHP-accredited Pharmacy Residency Completion of an Advanced Pharmacy Practice Experience (APPE) Equivalent pharmacy experience
- Strong clinical expertise in home infusion pharmacy
- Proficient in interpreting laboratory values and applying pharmacokinetics
- Excellent communication and patient counseling abilities
Schedule & Location:
- Monday through Friday, 7:00 AM to 3:30 PM
- Plano, TX
- Immediate start preferred
- 13-week assignment (contract)
