Staff Pharmacist in San Antonio, TX
This opportunity is seeking a Staff Pharmacist with expertise in sterile compounding to support home infusion therapy services. This 13-week contract position offers a consistent schedule from 12 PM to 8 PM, providing a focused work environment.

Schedule: 12 PM 8 PM, 13 weeks
Key Responsibilities:
- Evaluate patients for eligibility in home infusion therapy and coordinate care plans accordingly.
- Develop, monitor, and adjust individualized medication therapy based on clinical assessments, lab results, and pharmacokinetics.
- Prepare, verify, and dispense sterile medications compliant with USP standards, state and federal regulations, and pharmacy policies.
- Educate patients, caregivers, and healthcare providers on medication therapy and infusion administration techniques.
- Collaborate with healthcare professionals to ensure seamless continuity of care and monitor patient outcomes.
- Manage infusion pumps and medication administration devices, maintaining high standards of quality and safety.
- Provide accurate drug information, maintain thorough documentation, and support regulatory compliance and medication safety initiatives.
Qualifications & Desired Experience:
- Doctor of Pharmacy (PharmD) degree or graduate from an accredited School of Pharmacy.
- Active and unrestricted Texas Pharmacist license.
- Minimum of one year sterile IV compounding experience, or experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM). Alternatively, completion of an ASHP-accredited Pharmacy Residency or Advanced Pharmacy Practice Experience (APPE) or equivalent pharmacy experience.
- Strong clinical knowledge in home infusion pharmacy.
- Proficient in interpreting laboratory values and applying pharmacokinetic principles.
- Excellent communication skills with a focus on patient counseling and customer service.
