Home Infusion Staff Pharmacist in Schertz, TX
This position is for a Sterile Compounding Pharmacist providing home infusion services, based in Schertz, TX. This 13-week contract role offers a consistent schedule from 12:00 PM to 8:00 PM.

Key Responsibilities:
- Evaluate patients for eligibility and appropriateness of home infusion therapy.
- Develop, monitor, and adjust customized medication therapy plans using clinical assessments, laboratory data, and pharmacokinetic principles.
- Prepare, verify, and dispense sterile medications following USP guidelines and regulatory requirements.
- Educate patients, caregivers, and healthcare providers on medication administration, infusion procedures, and safe use.
- Collaborate with healthcare teams to ensure continuity of care and monitor treatment outcomes.
- Manage infusion pumps and medication administration equipment while ensuring safety and quality assurance.
- Provide drug information, maintain accurate documentation, and assist with regulatory compliance and medication safety initiatives.
Qualifications and Desired Experience:
- Doctor of Pharmacy (PharmD) or graduate from an accredited School of Pharmacy.
- Active and unrestricted Pharmacist license in Texas.
- At least one of the following:
- Minimum one year of sterile IV compounding experience.
- Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM).
- Completion of an ASHP-accredited Pharmacy Residency.
- Completion of an Advanced Pharmacy Practice Experience (APPE).
- Equivalent pharmacy experience.
- Proficient clinical knowledge of home infusion pharmacy settings.
- Ability to interpret lab values and apply pharmacokinetic concepts effectively.
- Strong communication, patient counseling, and customer service skills.
