LTC Mail Order Staff Pharmacist - Waukesha, Wisconsin

A pharmacy professional is sought for a Contract-to-Hire role specializing in Long Term Care (LTC) and Mail Order pharmacy services. This position is based in Waukesha, WI, with a schedule of Monday through Friday, 9:00 AM to 5:30 PM, including one weekend shift per month. The assignment spans approximately 13 weeks, with the potential for transition to a direct hire opportunity.

Key Responsibilities:

• Accept and dispense prescription medications accurately.
• Monitor patient medication compliance and detect over-usage patterns to ensure legal and safe supply.
• Verify that prescribed medications are appropriate and safe for patients.
• Maintain confidential, accurate patient records and pharmacy documentation.
• Counsel patients regarding medication usage, possible reactions, and answer questions.
• Offer guidance on prescription and over-the-counter medications, including dosage, storage, side effects, and interactions.
• Provide patient services such as smoking cessation support, blood pressure checks, and cholesterol management.
• Educate patients on the use and care of medical devices like blood pressure monitors and glucose meters.
• Compound medications for external and internal use and prepare sterile solutions following established protocols.
• Supervise pharmacy staff, including technicians and clerks, ensuring efficient pharmacy operations.
• Manage inventory of pharmaceutical and medical supplies.
• Identify drug-related problems, collaborating with healthcare providers to optimize patient outcomes.

Qualifications & Experience:

• Valid Wisconsin pharmacy license is required.
• Experience working as a pharmacist in a pharmacy setting.
• Long Term Care pharmacy experience is highly desirable.
• Proficiency in sterile compounding is necessary.
