Home Infusion IV Sterile Compounding Staff Pharmacist | Yukon, Oklahoma
A healthcare opportunity is available for a pharmacist specializing in home infusion pharmacy, supporting patients in the comfort of their homes through expert drug therapy management and sterile compounding practices. Location: Yukon, OK
Schedule:
- Monday through Friday, 8:30 AM 6:00 PM (rotating shifts including 8 AM5 PM and 9 AM6 PM options)
- Scheduled on-call shifts, typically once a month, Friday 5:30 PM Monday 8:30 AM
Duration: 13 weeks with potential for permanent hire
Key Responsibilities:
- Collaborate with physicians, nurses, and other healthcare professionals to assess and monitor drug therapy tailored to home care patients.
- Ensure compliance with all applicable state and federal pharmacy regulations and organizational policies.
- Participate in quality assurance programs and maintain accreditation standards.
- Manage new patient referrals and admissions, thoroughly reviewing and verifying all orders.
- Assist in planning patient care, setting therapeutic goals, monitoring drug therapy efficacy, and preparing progress reports.
- Perform initial and ongoing assessments to identify drug-related problems or therapeutic needs.
- Monitor patient outcomes by evaluating laboratory and clinical data.
- Provide consultation to patients, families, and healthcare teams regarding medication use.
- Perform sterile compounding of IV admixtures including parenteral nutrition, antibiotics, chemotherapy, and narcotics following aseptic techniques and USP 797 and USP 800 standards.
- Maintain thorough documentation of clinical activities and communication.
- Ensure compounding areas and equipment meet cleaning and safety standards.
- Keep current with pharmacotherapy knowledge through continual education and training.
Qualifications and Experience:
- Active Oklahoma pharmacy license required.
- Proven experience in sterile compounding.
- Experience in home infusion pharmacy or inpatient hospital settings.
