A Patient Referral Coordinator position is available in Syracuse, NY, offering a direct hire opportunity with a hybrid work schedule. This role requires a detail-oriented professional to manage the referral process for infusion and enteral therapies, ensuring smooth coordination between patients, healthcare providers, and internal teams.

Schedule:

• Monday to Friday, 8:30 AM 5:00 PM EST
• Hybrid work model: 3 days onsite and 2 days remote, following onsite training

Key Responsibilities:

• Process new infusion and enteral referrals from intake through initial handling
• Collect all necessary documentation and verify insurance coverage
• Obtain and track insurance authorizations to facilitate patient access to therapies
• Communicate coverage details and financial responsibilities clearly with patients and their representatives
• Collaborate effectively with referral sources, hospital liaisons, pharmacy, nursing, billing, and branch teams
• Maintain accurate patient records, referral logs, and authorization tracking systems
• Identify and recommend improvements to referral and intake workflows to enhance efficiency

Qualifications:

• Associates degree in Business, Healthcare Services, or a related discipline
• Minimum of 1 year experience in a healthcare environment
• Familiarity with payer requirements, billing procedures, insurance authorizations, and coverage specifics
• Strong computer, documentation, organizational, and multitasking abilities
• Excellent communication and problem-solving skills
• Preferred: 2+ years of experience in home care or infusion services
